Usual GP for All Patients


From 1st April 2015 all GP practices are required to provide all patients with a usual GP who will have overall responsibility for the care and support that the surgery provides to them.


All of our patients have a usual GP with overall responsibility for their care and support. Patients are not prevented from seeing any GP in the practice as they always have done.

Patient Access

White Lodge Medical Practice is now signed up to the Patient Access service. This allows you to access the practice online, and provides a number of features such as:

  • booking appointments online
  • cancellling appointments online
  • updating contact details
  • requesting repeat prescriptions

To register with the service, click this link. If you are struggling to register online, you may come into the surgery, where a receptionist can print off your personal PIN and Access ID.

HSCIC care.data programme


NHS England is commissioning a data service from the Health and Social Care Information Centre, called care.data. This programme will extract data from GP practices every month, to cover the period from April 2013 onwards. This data will help researchers identify patterns in disease and the most effective treatments.


Identifiers to be extracted:

  • NHS number
  • Date of birth
  • Postcode
  • Gender

No free text will be extracted; only coded clinical information such as referrals, prescriptions and diagnoses.

Your SCR is an electronic record, containing information about your medication, allergies and any adverse reactions you may have had to medication. This will make it easier for healthcare workers to treat you in an emergency, or if your GP practice is closed. You may also opt out of this service; to do so again please complete a separate opt-out form, or complete the form found here and return to the surgery.
